Title: Service Leader for Acute & Extended Care Services (Category I) Grade: 14

Departments: NCF/SNF, Acute Care, CCU, OB Status: Exempt

Reports To: Senior Vice President of Patient Care Services


Basic Function of Position:


The Service Leader for Acute & Extended Care Services is responsible for leadership, coordination, and integration of services in the clinical areas of NCF/SNF, Acute Care, CCU and OB and oversight of swing bed service.

As director he/she has authority and responsibility for development, in conjunction with other department leaders, policies and procedures that address the mechanism for assessing, meeting, and evaluating patient needs and the implementation of the hospitals plan for providing quality services to customers served. This will include ongoing programs to monitor, evaluate, and improve services provided.


Duties and Responsibilities:


  Responsible and accountable for the provision of high quality care in all designated areas following established policies and procedures.

  Demonstrates understanding of the physical and emotional changes in patients and can assure care is appropriate.

  Assures unit meetings and education programs are held as needed.

  Responsible for overview of department budgets, monthly statistics and generates reports for distribution to State for NCF/SNF.

  Supervises and assures schedules are completed.

  Assures adherence to hospital Infection Control policies.

  Corrects and/or reports any unsafe conditions.

  Is responsible for assuring family and patient education is completed.

  Attends meetings that are appropriate to the units.

  Supervises and evaluates current employees of the NCF/SNF department and assures same in designated areas.

  Is aware of department resources and practices cost-saving measures accordingly.

  Demonstrates understanding and compliance with elder, adult and children abuse policies.

  Required to participate in community health projects if asked.

  Must be able to provide effective and efficient leadership

  Must be able to effectively develop and maintain effective interpersonal communication

  Responsible for human resource management


Organizational Relationships:


  Reports to the Senior VP of Patient Care Services.

  Requires frequent communication with patients, families, physicians, ancillary services and other hospital staff members.

Minimum Qualifications:


A.     Education and/or Experience:

  Graduate of an accredited school of nursing.

  Current licensure as a registered nurse in the State of West Virginia.

  Minimum of three to five (3-5) years registered nursing experience with demonstrated willingness to assist in any nursing department.

  Must demonstrate previous excellence in nursing care.

  Advanced education is appreciated.

  Current BLS required and maintained


B.     Skills, Abilities, and Competencies:


  Computer literacy, word processing and excellent typing skills.

  Must have the ability to supervise and organize others. A thorough knowledge of effective management practices is essential.

  Skill in establishing and maintaining effective working relationships with staff.

  Requires good interpersonal communication skills when in contact with public, patients and staff.

  Requires knowledge of medical terminology, the ability to make decisions and use good judgement.

  Requires multi-tasking, problem solving and decision making abilities.

  Requires the ability to maintain confidentiality of medical records and other sensitive information.

Physical Requirements:


  Requires standing and walking for extensive periods of time.

  Requires sitting at a computer terminal on an average of 4-5 hours a day.

  Subject to repeated bending, squatting and stooping.

  Exposure to all patients elements including communicable diseases and body fluids.

  Requires verbal ability to communicate with patients and hospital staff.

  Requires corrective vision and hearing within normal range.

  Requires manual and finger dexterity and eye-hand coordination for easy and skillful use of hands when working with patients and equipment.

  Requires ability to lift and carry items weighing up to 50 pounds.

  Subject to stress and fatigue.

  Requires ability to work where activities are characterized by frequent change.


Environmental Conditions:


  Some exposure to toxic substances, ionizing radiation, medicinal preparations and other conditions common to a clinical environment.
